Request for Funds and Classroom Stipend Request Forms

 Forms
Two forms are used for requesting PTA funds, the Request for Funds and the Classroom Stipend Request. All school staff, PTA committee chairs and committee members, PTA Board members, and other PTA volunteers can use these forms to request funding support or reimbursements for expenses incurred while supporting the school and/or PTA-sponsored events and activities.

Request for Funds: This form is used for all funding requests except for the classroom stipend reimbursement.  This form can be found at the bottom of this page or in the PTA box in the Hale School Office.

Classroom Stipend Request: This form is used by the teaching staff to request reimbursement of their annual classroom stipend only. This form is blue.  This form can be found at the bottom of this page or in the PTA box in the Hale School Office.

Process for Requesting PTA Funds
Please send the completed forms with receipts to the PTA Secretary via the folders located in the PTA box in the Hale School office.

The YELLOW folder is for the Request for Funds.
The BLUE folder is for the Classroom Stipend Request for teachers.

All requests will be reviewed at the next monthly PTA Board meeting. Meetings are held on the 2nd Tuesday of each month  at 8AM in the Hale School office.

Not all requests for funds are approved. If you are unsure if your request is something that the PTA will reimburse, please contact the Co-Chairs to discuss the items you want funded.
