Bus Pass Request Form
Complete the Bus Pass Request form below if your child is going to a location other than their usual stop (i.e. - a play date) at the end of the day on the bus. This form is not needed for parent pick up. A note should be sent directly to the teacher if your child(ren) will be parent pick up by you or another parent.
This form must be turned into the office right away in the morning in order for a bus pass to be completed. The pass will be delivered to the classroom mid-morning. Requests received after 9:00 a.m. are not guaranteed to be completed.
Please do not email this information to the teacher or school secretary in case they are out of the building for the day. Other staff members do not have access to their emails.
